Have you tested the soil?  Maybe zinc deficiency.

Saw this in Merritt Island last year.

I suspect some kind of mite.

It happens when the plant goes dormant or loses vigor and the growing tips stop and die back.  Then many tips regrow from that point.  I think its from cold weather or aphid damage here.  I have lot of little potted rootstocks that have done it and definitely didnt get hit with herbacide.
