Starfruit Tree struggling

My brother in law planted this Starfruit in Phoenix, Az back in September. Tree was full, leaves were dark green and it was full of flower buds that were ready to open. Since then it slowly began deteriorating, buds remained on tree but never opened, and leaves began to yellow with large brownish splotches.

Any thoughts or ideas?

What fertilizers or soil amendments or sprays were used?  What is the irrigation like?  What have the weather conditions been?
