I'd suggest a recut at hip height and formative pruning as described in this video. This will seem harsh but has been done successfully many times. The ideal formation lets a person step right up into the tree from a standing position as if stepping onto a ladder. Pruning every year immediately after harvest is what you can expect for the life of the tree. Everyone who fails to prune eventually regrets it, myself included.
The formative pruning of the main structural branches is the most important step, everything later depends on this to make the tree productive & easy to manage in harvesting, pruning, and setting fruit.
Pay close attention to the part from 8:00 minutes onward. When a resprout starts choose branches to keep so that they are evenly spaced like spokes on a wheel when viewed from above as seen in the video's animation.

If you decide to recut make a slightly sloped cut so that water will run off, remove top branches first to lessen the load before making the final cut, use a pruning saw to get a clean cut.
