Lemon tree dropping leaves

I have a lemon tree is 5-1-1 mix. It's been in that mix for about 1.5 years. Recently it started dropping green leaves. I'd say it's dropped about 1/3 of the green leaves in the last 3 months or so.

I water and fertilize regularly.

Any ideas on some possible causes?

I've grown a lot of container citrus and having healthy green leaves drop isn't a problem I remember seeing.  I don't have much advice here other than the usual suggestion to check the container for dry or overly wet areas of the potting medium.

Millet:
Has the tree out grown the container? Also look to see if the the bark chips has disintegrated into a mesh.

Thank you both. I checked the medium and it looks okay. It's not overly wet or dry, but I think previously, it may have dried out a little so I think that may have been it.

It's also not root bound.

I'll keep an eye on the soil and watering and see what happens.

Millet:
It is very difficult to over water a 5-1-1 medium to the point of causing damage to the tree.  5-1-1 mediums have a very high aeration level even after soaking in water for a while.
