Sorry I don't have this species any longer myself. Would be into finding it again too. Seemed very hard to keep alive long term. I have had this species around 10 times over the years and it eventually died every time. Best I ever did was straight perlite for soil but it seemed too picky for most any other soil. Had it going well a little over a year that particular time.

I had the same experience with a few different varieties of salvia, including some supposedly native ones. It performed like an annual plant for me. Even though it survived into subsequent years, it got weaker and stemmier and thinner looking. I tried to propagate it from cuttings to reinvigorate the flower bed, but that barely worked. Too much trouble for not enough flowers and not enough density. We have so many pollinator plants available, I just ended up choosing better options that take less work.
